The Manager at Church Street considers all referrals and responds to the referrer as appropriate. The referral procedure is of vital importance to the whole operation of  the unit as inappropriate arrivals can have a disastrous effect upon the existing residents and the establishment.

We insist on honesty from referring agencies, and access to all records prior to admission.  Pessimistic histories and suggestions of a poor prognosis will not necessarily deter us from accepting a young person.

The process should begin with a telephone enquiry and referral forms, together with any appropriate papers will be despatched immediately.  Upon receipt of the completed referral form, if the referral appears suitable, the Manager will arrange for a visit to be made to the Home where possible.

If we offer a placement the field social worker and/or relative will be given a set of contract papers to complete and sign.  No young person is admitted, under normal circumstances, without these being completed and returned.

At any stage in the procedure the field social worker, relatives or young people may make interim visits or request any other information.


ALL ADMISSIONS MUST BE PLANNED AND PREFERABLY A PRE- PLACEMENT MEETING HELD. Please note that an admission could take place the same day as the referral – in a planned format

 
The homes must be provided with all relevant paperwork on admission including:

 
·      A referral form.

·      A contract of placement.

·      All appropriate Looked After Children documents (LAC)

·      A planning meeting to proceed within 72hours
